When beginning a new project, please complete this brief form to the best of your ability. Having all details upfront in a single location saves time, emails, and delays in production. Once received, the Marketing and Communications team will confirm and follow up on next steps for your marketing and promotional project.

A community benefit activity is an event that educates the community about mental health or addiction, but does not promote Rogers’ treatment specifically. For more information, please see the CBA decision tree.

Process for requesting T-shirts

We are happy to help coordinate your order! To begin, review the process below and fill out the order form. Please plan on at least six weeks to receive your shirts. A PO# and sizes/quantities are required to place your order.

If you are planning a specific campaign or incentive that requires a custom design, please allow an extra two weeks.

• Please review the order form below and gather the information needed to fill it out.
• Our standard t-shirt is a cotton Next Level unisex t-shirt available in a variety of colors. If you would like a different style shirt, please let us know what you are looking for and Deb will send you options from our vendor.
• In order to provide a quote, we will need your final shirt selection, quantities, sizes and an approved design. Minimum order is 12 shirts.
• Once we provide you with a quote, you may create the PR.
• Once the PR is approved and a PO# is issued, Deb and Haley will coordinate your order with the vendor.
