Partners in Behavioral Health is an independent, professional organization that specializes in managing a full spectrum mental health network providing psychiatric and substance abuse care. We assure that cost-effective, quality behavioral health services are provided to our clients through partnerships with insurance companies, third party administrators, physicians, therapists, and employers for their employees and employee dependents.
PBH offers an extensive and comprehensive network of quality behavioral health care providers — psychiatrists, nurse practitioners, psychologists, hospitals and clinics who offer every specialty and level of care available. As a Wisconsin-based network, we are able to provide localized, high quality service to our members while keeping costs low.

**Please note that as of January 1st, 2021 our panel will be closed until further notice.
Clinics which are not currently in the PBH Network will be required to complete the Provider Agreement. This contract will need to be signed by the clinic owner/director or sole proprietor.
For billing purposes, each clinic will also need to provide banking information to Health Payment Systems. Note Health Payment Systems Letter and Banking Form. (HPS fax number is provided at the top of the form)
Each provider in a particular clinic will need to complete the Preferred Provider Application Survey as well as provide the required documents for credentialing. The required documents are: proof of insurance, a copy of state license, resume, as well as DEA for all Prescribers.
